New Family Registration Form

Welcome to The Garden Church Kids' Ministry! We ask each family to complete this form (one for each child) to help us know them better and care for them well. If you have any questions about our Kids' Ministry programs, policies, or procedures, please reach out to Pastor Aly Wilson at kids@thegardenboston.com.

There's no need to add an adult if we already have parent/spouse information for your family on file.
